Check list

Things you should bring with you when traveling to Poland:
- Documents:
- Valid passport (ideally with more than 3 months validity before expiry)
- Plane tickets
- European Health Insurance Card
- OPG/ CT guide (if you have one)
- Your dentist's treatment plan (if you have one)
- Any other relevant documents about your health (hospital treatment
etc.)
- Name, address, phone number of your hotel
- Name, address, phone number of INDEXMEDICA clinic
- Name, address, phone number of a friend/contact person in the UK
- Name, address, phone number of your physician in the UK
- Items:
- Mobile phone and charger
- 3-pin to 2-pin adapter (important - the electricity system in Poland is of
continental type!)
- Any medicines which you take on a regular basis e.g. inhalers, insulin,
allergy
pills, blood pressure tablets, etc.
- Cash/ credit and debit cards/:
- Cash
- Credit/debit card (with sufficient credit limit for your treatment, hotel
and
living expenses, and better take two cards, just in case!)
- Traveller's cheques (optional)
